Case Manager Youth Programs

About the role:

The Case Manager Youth Programs facilitates a range of support services for young people aged 12 – 25 years, providing case management, group programs and psychosocial education to young people with a lived experience of family, domestic, or sexual violence (FDSV).

In consultation with the Youth Programs Coordinator, the Case Manager Youth programs will:

  • Provide screening, assessment, and case management for clients to ensure safety and develop individual safety plans. Liaise with other service providers as needed.
  • Maintain continuous engagement with relevant service providers for accurate information and internal and external referrals for clients.
  • Work with the Youth Programs Coordinator to develop and facilitate group program content that engages young people in psychosocial education and support.
  • Manage client data, evaluations, and quality improvement in collaboration with the Youth Programs Coordinator, including maintenance of case notes for statistical data collection and external reviews.
  • Work towards, develop and maintain cultural awareness and fitness through participation in cultural training, discussions, and diversity initiatives.

 

The details: 

  • Part-time role
  • Fixed-term, 12-month contract
  • Located in Deakin, ACT
  • Annual salary between $95,557.70 to $99,848.89

 

What you'll need to succeed:

  •  Relevant experience and qualification in the field of Social Work, Psychology, Nursing, Occupational Therapy, Counselling or Social Science
  •  Demonstrated skills and experience in providing case management and an in-depth understanding of the impact of family, domestic and sexual violence on children and young people
  •  Demonstrated knowledge of theoretical frameworks for providing client centred and strengths-based intervention that is trauma-informed, and crisis intervention
  •  Demonstrated skills and experience in developing and delivering psycho-educational and/or therapeutic groups
  •  Knowledge and understanding of the National Principles for Child Safe Organisations and demonstrated ability to integrate principles into own learning and work practices
